Indications/Uses

Herpes simplex & varicella zoster infections. Herpes simplex infection prophylaxis in immunocompromised patients. Herpes simplex infections in neonate.

Dosage/Direction for Use

Adult Herpes simplex, varicella zoster infections & herpes simplex prophylaxis in immunocompromised patient 5 mg/kg every 8 hr. Herpes encephalitis, varicella zoster infections in immunocompromised patient & herpes simplex infection in neonate 10 mg/kg every 8 hr. Childn 3 mth-12 yr Herpes simplex, varicella zoster infections 250 mg/m² every 8 hr. Varicella zoster infection in immunocompromised childn, herpes encephalitis 500 mg/m² every 8 hr. CrCl 25-50 mL/min 5 or 10 mg/kg every 12 hr, 10-25 mL/min 5 or 10 mg/kg every 24 hr, 0-10 mL/min 5 or 10 mg/kg or 500 mg/m² should be halved & administered every 24 hr & after dialysis.

Contraindications

Hypersensitivity to acyclovir or valacyclovir.

Special Precautions

Renal impairment. Pregnancy & lactation. Elderly.

Adverse Reactions

Phlebitis; nausea, vomiting; reversible increased liver enzymes; pruritus, urticaria, rashes including photosensitivity; increased blood urea & creatinine.

Drug Interactions

Increased AUC by probenecid & cimetidine; mycophenolate mofetil. Monitor renal function changes w/ cyclosporine, tacrolimus.
